Sloan Comfort Care Home

Sloan Comfort Care Home was built through a partnership between the Accountable Health Community, Inc., a not-for-profit organization formed to address disparities in healthcare, and St. Andrew's Church in Sloan, NY.  The Home offers an alternative for hospitalized patients, who do not wish to die in an institution or for people in the community who for a multitude of reasons cannot receive end-of-life care in their own home. Located on the beautiful campus of St. Andrew's Church, a convent is currently being repurposed and renovated into a two-bed private home.
